What is the 'Asthma and Allergic Rhinitis questionnaire'?
It is a patient self-assessment tool that patients can use in discussions with their pharmacist, doctor or nurse.
Why should patients complete the questionnaire?
This is a simple questionnaire that will give pharmacists, doctors or nurses information about how well a patients Asthma and Allergic Rhinitis are managed, and provides a framework for asking further questions that could help the patients to manage their condition better.
Who created the questionnaire?
The questionnaire was created by 3 organisations:
- Education for Health - the UK's leading education charity for health professionals working with patients with long-term conditions
- Allergy UK - the countries leadings medical charity dealing with allergy
- Asthma UK - the charity dedicated to improving the health and well being of the 5.4 million people in the UK whose lives are affected by Asthma
The campaign was initiated and funded by MSD.
